Operation Smile - Survey - ISLC 2024 Travel Form

ISLC 2024 will be held at Emory University in Atlanta, Georgia, USA from July 22-28, 2024. Participants must book their own travel to and from Atlanta, Georgia, USA. All travel must arrive at Hartsfield–Jackson Atlanta International Airport (ATL).

OSI will provide ground transportation from Hartsfield–Jackson Atlanta International Airport (ATL) to Emory University on Monday, July 22, 2024. Please ensure that your flight will arrive before 3:00 pm (15:00) EST.

OSI will also provide return transportation to the airport on Sunday, July 28, 2024. The last bus will leave for the airport at 12:00 pm EST .

If your travel plans are to arrive or depart outside of those time frames, please contact the Student Programs team prior to confirming travel purchases for review and assistance at islc@operationsmile.org.

Please note that some participants may be required to obtain a visitor’s visa to enter the USA. Please visit https://travel.state.gov/content/travel/en/us-visas.html/ to review if a visa will be required for travel to USA.

After completing this form, please email ISLC@operationsmile.org with a copy of your travel itinerary, passport bio page, and visa (if required).
